Media Release Form

The undersigned hereby grants Tiger Athletic Foundation a private Louisiana nonprofit corporation, a non-exclusive royalty-free world-wide license to utilize his/her name and image in connection with promotional materials including but not limited to videos and brochures produced and displayed by TAF to assist in fundraising activities benefiting TAF and LSU Athletics; provided, however, that copies of promotional materials utilizing the undersigned’s name and image shall be provided to the undersigned or his or her designee for review and approval at least seven (7) days in advance of publication. In connection with this release the undersigned represents and warrant the following statements are true and correct:

• I have not received, I have not been promised, and I will not receive any consideration or benefit, financial or otherwise, in return for granting this release; and;
• My sole motivation in granting this release is to promote membership in and assist TAF in raising private funds to support the LSU Athletics program.

This release may be revoked by the undersigned by giving thirty (30) days advance written notice to Tiger Athletic Foundation, Attn: President & CEO, PO Box 711, Baton Rouge, LA 70821, with a copy to LSU Athletics Department, Attn: Director of Athletics, LSU Athletic Administration Building, Baton Rouge, LA 70803.
